Why Is Rafael Stock Falling Wednesday?

Rafael Holdings Inc. (NYSE:RFL) stock is trading lower on Wednesday after the company shared topline efficacy and safety findings from its subsidiary Cyclo Therapeutics LLC‘s pivotal Phase 3 TransportNPC study of Trappsol Cyclo in Niemann-Pick Disease Type C (NPC).

The micro-cap biotech company had a market capitalization of about $105.6 million at the previous close, making the stock particularly vulnerable to sharp swings following clinical trial updates.

Trading activity surged as investors reacted to the data. About 1.90 million Rafael shares changed hands during the session, more than 12 times its 100-day average volume of roughly 155,467 shares, according to Benzinga Pro data.

Study Misses Endpoint

While the drug demonstrated a directionally favorable treatment effect by slowing disease progression by 64% at Week 96, the overall primary endpoint did not reach statistical significance.

However, a prespecified subgroup analysis revealed a statistically significant benefit, keeping the drug developer on target for a regulatory filing later this year.

The 96-week trial evaluated 94 patients aged 3 to 70 who received either 2000 mg/kg of intravenous Trappsol Cyclo every two weeks or a placebo.

In a prespecified subgroup representing 83% of participants on background miglustat or leucine therapy, the candidate slowed disease progression by 71% compared to placebo, achieving statistical significance.

Phase 3 TransportNPC Trial Key Outcomes

The treatment exhibited a well-tolerated safety profile consistent with underlying NPC symptoms.

Treatment-emergent adverse events affected 93.8% of the Trappsol Cyclo cohort versus 100% in the placebo group. Serious treatment-related adverse events occurred at similar rates between the drug group (3.1%) and placebo (3.3%).

Hearing-related events affected 15.6% of treated patients and 13.3% of placebo patients, with one severe case of bilateral deafness reported in the treatment arm.

One patient discontinued treatment early due to an adverse event, and no deaths occurred during the trial.

Safety Profile And Overall Survival Findings

A separate overall survival study compared infantile-onset NPC patients against real-world controls.

Trappsol Cyclo administration correlated with an 85% lower mortality risk compared to matched registry controls. Expanding the historical control cohort to include additional published studies revealed a 94% reduction in mortality risk.

Following a prior pre-NDA meeting with the FDA, the company expects to submit a New Drug Application in the fourth quarter of 2026.
