Rezolute Inc. (NASDAQ:RZLT) shares initially traded higher Monday before pulling back and turning negative.

The company announced that following a preliminary review of detailed data from the Phase 3 sunRIZE study of ersodetug for hypoglycemia due to congenital hyperinsulinism (HI), the Food and Drug Administration (FDA) recommended a pre-BLA meeting.

Hypoglycemia due to congenital HI is a rare genetic condition where the pancreas releases too much insulin regardless of blood sugar levels, causing severe and persistent low blood sugar in infants.

Navigating Missed Clinical Trial Endpoints

Previously, the clinical-stage company disclosed that the Phase 3 sunRIZE study failed to achieve its primary endpoint.

The recorded reductions in hypoglycemia events, tracked through finger-stick self-monitored blood glucose (SMBG) methods, lacked statistical significance when evaluated against a placebo group.

Additionally, the trial did not meet its key secondary endpoint regarding the time patients spent in hypoglycemia, measured by continuous glucose monitoring (CGM) during the Week 24 evaluable window.

Despite these missed targets, researchers observed consistent and clinically relevant improvements across multiple other CGM-focused hypoglycemia endpoints within both treatment arms.

During a Type B meeting held in March 2026, the FDA recognized the behavioral challenges inherent in clinical trials for this specific patient population.

Regulators specifically noted the limitations surrounding SMBG-based measurements.

As a result, the agency requested independent access to the continuous CGM data. Rezolute subsequently submitted comprehensive source datasets, summary results, and post-hoc analyses in June 2026.

Future BLA Strategy and Ongoing Extension Phase

After thoroughly reviewing the submitted information, the FDA encouraged Rezolute to request a Type B pre-BLA meeting as a preliminary step toward submitting a Biologics License Application for congenital HI.

The regulatory agency explicitly stated that this encouragement does not guarantee final approval, noting that authorization depends on an in-depth evaluation during the formal BLA review process.

Looking ahead, Rezolute anticipates releasing topline results from its upLIFT study concerning tumor HI later this quarter. If these upcoming results support a submission, the company might secure an additional pre-BLA meeting.

Management plans to finalize an overarching BLA submission strategy for both clinical programs once the upLIFT data becomes public.

Meanwhile, the open-label extension phase of the sunRIZE study remains underway with strong patient retention.

Treatment durations now stretch from nine months to more than two years.

Participants demonstrate sustained glycemic control and a significant drop in their reliance on background standard-of-care therapies, suggesting potential long-term clinical benefits from continued ersodetug usage.
